Email your signed and scanned pdf NFS I-K USDA reports directly to:
CEL-INKIND@cityofchicago.org each month using the following format for the subject of the email:
Notes for Non-Federal Share (NFS) In-Kind (I-K) Matching Contribution/USDA Food Program Report submission:
• Reports are due monthly, no later than the 10th of the following month.
- Remember to update your report period and report # with each monthly submission.
- If you are submitting a revision to a prior period, please mark the revised submission box and indicate an “R” after the report #.
• Delegate agencies shall only report NFS/I-K costs that are listed in the approved delegate agency budget for each program.
• Late submission or non-sufficient NFS/I-K contributions can result in your invoice payments being placed on hold.
